#53: Heart Failure Diagnosis and Management with Dr. Gorodeski

Episode Summary

A heart failure diagnosis can sound frightening, but it does not mean the heart has stopped working—or that recovery is impossible. Dr. Chris Huff talks with advanced heart failure specialist Dr. Eiran Gorodeski about symptoms, causes, modern treatments, and the evolving future of heart failure care.

Episode Notes

Heart failure is a serious diagnosis, but it is also highly treatable. In this episode, Dr. Chris Huff welcomes Dr. Eiran Z. Gorodeski, a leader in the treatment of advanced heart failure, to explain what heart failure actually means—and why the word “failure” often gives patients the wrong impression.

They distinguish heart failure with reduced ejection fraction (HFrEF) from heart failure with preserved ejection fraction (HFpEF), explore common causes including coronary artery disease, high blood pressure, and cardiometabolic disease, and discuss less familiar conditions such as inherited cardiomyopathies and cardiac amyloidosis. Dr. Gorodeski describes symptoms worth recognizing and how clinicians use medical history, examination, imaging, and laboratory tests to identify the condition and its underlying cause.

The conversation also covers the four pillars of guideline-directed medical therapy, the importance of specialized care teams, individualized approaches to diuretics, nutrition, and exercise, and when advanced therapies such as mechanical heart pumps or transplantation may be considered. They close with emerging possibilities—from AI-assisted diagnosis to fully implantable devices and xenotransplantation.

What You’ll Learn

Key Takeaway

A heart failure diagnosis is not necessarily a final chapter. With a careful diagnosis, evidence-based treatment, appropriate exercise, and coordinated follow-up, many patients can feel better, avoid hospitalization, and live longer—and some people with weakened heart muscle experience meaningful improvement in heart function.

About the Guest

When most people hear “heart failure,” they hear an ending. Dr. Eiran Gorodeski has built his career on writing the next chapter.

His path started at Brandeis University, where he graduated magna cum laude, and continued at Case Western Reserve University School of Medicine, where he was inducted into the Alpha Omega Alpha honor society. He trained in internal medicine at Massachusetts General Hospital, then came back to Cleveland for cardiovascular training and an advanced heart failure and transplant fellowship at Cleveland Clinic.

He spent nearly a decade as a staff cardiologist there, caring for the sickest hearts, including patients on mechanical support and awaiting transplant. Along the way he saw that the hardest moments often came after discharge, in the handoffs between hospital, rehab, and home. So he went deeper. He earned a Master of Public Health from Harvard, studied health care delivery at Harvard Business School, ran Cleveland Clinic’s post-acute care operations, and stepped

in as interim director of its palliative medicine program.

Today, Dr. Gorodeski is Medical Director of the Advanced Heart Failure & Transplant Center and the James and Angela Hambrick Center for Cardiac Recovery at University Hospitals’ Harrington Heart & Vascular Institute, and Professor of Medicine and Section Head of Advanced Heart Failure at Case Western Reserve University. He holds the Daniel I. Simon, MD, Endowed Chair in Cardiovascular Excellence. His research focuses on heart failure care in the U.S. and novel care models for older adults living with the disease. This year, he chaired the writing committee for the HF STATS 2026 report, the Heart Failure Society of America’s comprehensive, annually updated reference for heart failure epidemiology and statistics in the United States.
